SvelteKit 3

[Rich-Harris]

This adds support for SvelteKit 3:

• replaces $app/environment with $app/env
• replaces the store subscription with afterNavigate
• updates the demo app

It supersedes #214 and #215, and would require a major version bump as it no longer supports SvelteKit 1 or 2.
