Annotate ADR 0048 and ssh/CLAUDE.md: av's block at the top of ~/.ssh/config sets IdentityAgent for all hosts, so agent ssh no longer needs a 1Password approval. Co-Authored-By: Claude Sonnet 5.5 <noreply@anthropic.com>

~/.ssh/config now opens with an av-managed Automic Vault block that sets IdentityAgent for every host. Agent ssh no longer needs a 1Password approval, so ADR 0048 and ssh/CLAUDE.md get a dated note saying so. The old text stays as history.
