In Issue #41657, I reported missing conversation history on Windows. A scan of 15 source record files found two duplicate-ordinal cases while the underlying records remained intact. I shared diagnostic evidence and proposals covering writes, ordinal management, projection rebuilding and regression checks. Other Windows and macOS users also supplied evidence of similar failures in the comments.
