Issues

• context-graph: a hook blocked on Memgraph is killed at the runtime's hook timeout and leaves no capture-failure record

• skills-graph: :Skill identity is the bare name, so same-named skills from different scopes merge

memgraph/ai-toolkit#634 · Filter by author mkolodzej opened on Oct 28, 2026

• context-graph: Codex tool results never get is_error, so failed Codex commands are stored as completed

memgraph/ai-toolkit#633 · Filter by author mkolodzej opened on Oct 28, 2026

• sessions-graph: every embed process re-runs schema DDL and drops the embedding on a storage-access TransientError

• context-graph: Antigravity CLI sessions never leave status in_progress (no session-end hook is wired)

memgraph/ai-toolkit#631 · Filter by author mkolodzej opened on Oct 28, 2026

• context-graph: Antigravity CLI hooks generated on Windows always fail (POSIX single-quoted exe path through cmd.exe), aborting every tool call

• context-graph: rolling back to native fans every user-wide memory into every Claude project folder (29,922 files on one host); Codex gets nothing back

memgraph/ai-toolkit#629 · Filter by author mkolodzej opened on Oct 28, 2026

• context-graph: setup --memory-backend has no --home, and --plan doesn't list the files the memory step rewrites

memgraph/ai-toolkit#628 · Filter by author mkolodzej opened on Oct 28, 2026

• context-graph: Codex memory rollback leaves an empty [memories] table and reuses a stale .context-graph.bak as the before-state

memgraph/ai-toolkit#627 · Filter by author mkolodzej opened on Oct 28, 2026

• context-graph: config and memory writes use Path.write_text without newline=, so Windows files flip to CRLF

memgraph/ai-toolkit#626 · Filter by author mkolodzej opened on Oct 28, 2026

• context-graph: session-start memory index is capped in lines, not bytes, and lists every root file (~41 KB per session after a user-wide import)

memgraph/ai-toolkit#625 · Filter by author mkolodzej opened on Oct 28, 2026

• context-graph: Claude Code memory import keys Windows projects by slug, so session start never lists them

memgraph/ai-toolkit#624 · Filter by author mkolodzej opened on Oct 28, 2026

• context-graph: subagent actions land on the parent Session when subagent lifecycle events are missed
