What changed

Add transactions().subscribe_failures() so diagnostic consumers can observe failed live transactions without taking over the processed stream. Notifications reuse the full transaction payload, including the structured error, slot, account/program addresses, instructions, and available execution recordings.

Also track the repository instructions with reader-focused documentation rules and include the accompanying CPI cleanup.

Closes #147

Impact

  • Failure subscriptions use bounded multicast. Overflow disconnects the slow receiver and increments the existing disconnect metric; resubscription observes future events only. Admission rejections, simulation, and replay are excluded.
  • Processed and account-log subscriptions now return Arc<FullTransaction>. Their existing single-consumer and successful-transaction selection contracts, respectively, are unchanged.
  • Empty subscription classes use atomic checks and skip notification allocation, account snapshots, and extra balance copies. Keyed fanout has no separate presence lookup.

Reviewer notes

Borrowed account images are materialized once before sharing, while execution ownership still protects their backing storage. Ledger projection preserves metadata needed by live consumers.

Local formatting and strict all-feature/all-target Clippy checks passed for Engine, Keeper, Processor, and Ledger, along with seven existing focused tests covering fanout, admission, execution failure, resource limits, simulation, replay, and cancellation. No new tests were added; throughput and MBV integration were not validated. The custom WebSocket endpoint and CLI monitor remain outside this Engine change.

📝 Walkthrough

Keeper now exposes failure-only transaction subscriptions and sends shared full transactions through processed and account-filtered log subscriptions. Execution commit handling checks execution outcomes and subscriber availability before preparing notification details. Account updates and TLS messages remain tied to successful state transitions. The change also updates repository guidance, a test borrow, and schema documentation.

The change is mergeable with a bounded follow-up: add a focused test confirming that failed live transactions reach the new subscription and successful ones do not.

Explanation

The subscription implementation and its tests are in scope for [#147]. The .gitignore change, the new repository-level AGENTS.md instructions, and the documentation-only ledger/src/schema.rs CPI cleanup have no demonstrated connection to the failure subscription objective. The linked issue excludes unrelated consumer work, and these changes do not implement that objective.

Resolution

Remove the unrelated .gitignore, AGENTS.md, and ledger/src/schema.rs changes from this pull request, or move them to a separate pull request. Keep only the failure-subscription implementation and directly supporting tests and refactoring.
