[QTI] The 'Add distractor' button is being used for two different functions

Observed behavior

For a question of type 'Associate' when I click the 'Add distractor' button it opens the text editor and when I input a distractor item and then I close the question the text is lost.

I have to click again the 'Add distractor' button in order to actually add the first one while seeing an empty editor for the second which is confusing:

Steps to reproduce the issue

1. Sign in to Studio at unstable
2. Go to Channel > Channel editor > New exercise
3. Create an exercise and add a question of type 'Associate'
4. Add matching pairs and then attempt to add a single distractor item
