Fixes two issues in the hosted demo on App Engine Standard:\n1. Normalizes $_SERVER['SERVER_NAME'] from $_SERVER['HTTP_HOST'] in the examples/index.php front controller (App Engine Standard's internal serve nginx sets SERVER_NAME to localhost/"", causing hostname-mismatch during verification).\n2. Restores isset($_POST[ReCaptcha::RESPONSE_KEY]) (matching recaptcha-v2-checkbox-explicit.php and recaptcha-v2-invisible.php) across recaptcha-v2-checkbox.php and the explicit transport examples so submitting without completing the checkbox displays missing-input-response.
