Saved-default hint only when the saved address is what failed - #40

flash-self reads the default connection, then flashes over USB DFU and waits for the USB console; when that console did not come back, the error got the "saved default connection" hint although the saved address was never dialed. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

Seen flashing a new pod:
flash-selfflashed over USB DFU, the USB console did not come back (expected on a first ROM-bootloader flash), and the error ended with "10.237.8.85:8080 is the saved default connection...", although that address was never dialed.The hint (from #38) now needs the failing error to name the saved address. Test case added for the USB path.
