All exercises suddenly dropped from assignment

Describe the bug
I went back and regraded all assignments in the course so that students with late work could get credit. There are over a dozen assignments and all worked fine except the CH 4 Lab Exercises assignment suddenly and inexplicably dropped all of the exercises on the assignment. The regrade resulted in students all getting 0's.

I managed to recover the exercises (at least 4 of them—I don't remember how many there were to begin with) because I always associate exercises I create with the chapter. So I just went back and readded the exercises to the assignment, and then regraded. Since student solutions remain associated with the exercise, it appears to have retained their solutions and scores are (hopefully) restored.

There is no code anywhere in the system that would spontaneously delete questions from an assignment without someone hitting a button to delete them, especially when you are the one who entered them in the first place. I don't think I've ever heard of this happening before.

Assignments use the id number of the question in the questions table and the id number for those four questions was assigned on September 12 and has not changed.

Can you think of any action you may have taken that would modify these exercises or this assignment in any way?

So, this is quite a mystery. I hate mysteries! But I really like to get to the bottom of them!
